Can a Counsellor Help My Depression, or Do I Need Medication From a Doctor?

Short answer: For mild to moderate depression, structured psychotherapy such as cognitive behavioural therapy is an effective standalone treatment and no referral is needed to access it in Alberta. What therapists can and cannot do

Psychologists and counsellors in Alberta do not prescribe medication. A therapist can provide psychotherapy, help you track symptoms, and support conversations with your physician, but the prescribing decision belongs to a physician, psychiatrist or nurse practitioner. Formal diagnostic assessment falls within the psychologist scope of practice.

A therapist should never advise you to stop or change prescribed medication. If a therapist does, treat that as a serious concern and speak to your prescriber.

What therapy for depression actually involves

Effective depression therapy is more active than most people expect. It generally includes:

  1. Behavioural activation. Rebuilding activity before motivation returns, because in depression motivation follows action rather than preceding it. This is one of the most reliably effective components.
  2. Cognitive work. Identifying and testing the depressive interpretations that maintain low mood, rather than simply thinking positively.
  3. Sleep, routine and physical health. Restoring the daily structures that depression dismantles.
  4. Addressing what maintains the episode. Grief, isolation, relationship conflict, burnout, unresolved trauma or chronic self-criticism.
  5. Relapse prevention. Recognising early warning signs and having a plan, which is where therapy's durability advantage comes from.

How long treatment takes

A structured course of therapy for mild to moderate depression commonly runs 12 to 20 sessions. Some improvement is often noticeable within the first several weeks, particularly in activity level and sleep. Antidepressant medication also commonly takes several weeks to reach full effect, so neither route produces immediate change and stopping early is the most common reason either appears to fail.

You do not need a diagnosis to start therapy

Counselling in Alberta is accessed directly, without a physician referral or a formal diagnosis. Many people begin therapy while still unsure whether what they are experiencing qualifies as depression. Uncertainty is a legitimate reason to book, not a reason to wait.

Will a counsellor tell me to stop my medication?

No. Counsellors and psychologists do not make prescribing decisions and should not advise changing medication. Therapy is routinely delivered alongside medication.

Is therapy or medication more effective?
